Please read and fully understand and acknowledge the following prior to completing a signed declaration at Lucky 13 Tattoo which will enable us to proceed and administer your tattoo.

1. I have been fully informed with the inherent risks associated with getting a tattoo. I fully understand that these risks known or unknown, can lead to injury. including but not limited to infection, keloid / scarring and allergic reactions to latex gloves and or soap.

2. Having been informed with the potential risks of getting a tattoo, I still wish to proceed with the tattoo and I freely accept, and expressly assume, any and all risks that may arise from the tattoo.

3. I waive and release to the fullest extent permitted by law, the Tattoo Artists from all liability for any and all causes of action that I, my estate, heirs, executors or assigns for personal Injury or otherwise. This includes any direct, and/or, consequential damages which result or arise from the tattoo whether caused by neglect or fault of the Tattoo Artist, studio or otherwise.

4. The Tattoo Artist has given me the full opportunity to ask any and all questions about the tattoo procedure. The artist has answered these questions to my total satisfaction.

5. I acknowledge that the Tattoo Artist has given me instructions on the aftercare of my tattoo whilst it is healing, and I understand and agree to follow them.

6. I acknowledge that it is possible that the tattoo can become infected, particularly if these instructions are not adhered to.

7. I acknowledge that I have informed the Tattoo Artist that I do not have diabetes, epilepsy, haemophilia, a heart condition, use blood thinning medication, skin conditions, pregnant or nursing or suffer any other medical condition that could interfere with the procedure or healing of the Tattoo. I am not the recipient of an organ or bone marrow transplant and if I am, I have taken the prescribed regime of antibiotics that is required by my Doctor or Specialist in advance of any invasive procedure, such as a tattoo.

8. I acknowledge that the tattoo will result in a permanent change to my appearance and that can or may be removed by laser or surgical means which can be disfiguring and costly and in all likelihood, will not result in the restoration of my skin to its exact appearance prior to being tattooed.

9. I acknowledge that I am SOLELY responsible for ensuring that my tattoo has the correct spelling and grammar and where applicable, any symbols or icons have the correct meaning. I understand that I may review my design and make any changes at any stage PRIOR to the application of the stencil. I may request to have the stencil repositioned until I am certain about the placement.

10. Variation in colour and design may exist between the art selected and the actual tattoo after it has been applied to my body. I also understand that the colours and clarity of my tattoo will deteriorate due to exposure to the sun, fluctuating body weight and naturally occurring dispersion of pigment under aging skin.

11. I understand that moles, stretchmarks, birthmarks and other skin abnormalities, such as scabs or sun burn, may be an irritant or hazard to the application of my tattoo. I acknowledge that the Tattoo Artist will proceed based solely on their experience and that I may be required to seek specialist advice. If I choose to proceed regardless, I am responsible for any adverse consequences.

12. I acknowledge during the tattoo and healing process that it is common for the tattoo to fade, drop out or otherwise reject and that I am entitled to ask for a free touch-up. I acknowledge the necessity of a touch-up can only be determined by the Tattoo Artist or the Director of Lucky 13 Tattoo. I have been made aware that touch-ups are free with the exclusion of hands, feet and other known problem areas. If any touch-ups are required due to my own negligence, the work will be done at my own expense.

13. I hereby declare that I am of legal age (with valid proof of age provided) and I am competent to sign this agreement

14. I acknowledge that I am not under the influence of alcohol and or drugs and that I am voluntarily receiving a tattoo without

15. I understand that no form of anaesthetic will be used in this tattoo procedure.

16. I understand that any and all payments towards my procedure including deposits will not be refunded.

17. I understand that LUCKY 13 TATTOO takes no liability for requiring the help of emergency services in aiding our clients in the case of fainting or other incidents within their premises.

18. I understand that EVERY care will be, taken to ensure the tattoo procedure is carried out in a hygienic manner including disposable and pre-sterilised equipment (Australian Standards 4187).

I release all rights for photographs to be taken of me and my tattoo and consent their reproduction in print or electronic format for use by Lucky 13 Tattoo only.

I acknowledge that if I cancel my appointment I forfeit the deposit and the design created for me.

I acknowledge that I have been given adequate opportunity to read and understand this document and I agree to be bound by it.
